1. Who we are and what this policy covers

Excel Medical Associates is a primary care clinic located at 15490 NW 7th Ave #101, Miami, FL 33169. In this policy, "we" refers to the clinic and "you" to anyone visiting this website.

This policy applies only to this website: to the information you type into our forms and to the basic technical data your browser generates when you visit. It does not apply to our social media pages, to third-party sites we link to, or to the care you receive in person at the clinic.

2. This document is not the HIPAA Notice of Privacy Practices

Excel Medical Associates is a HIPAA covered entity. That means your medical record and the health information you share during your care are protected by the clinic's Notice of Privacy Practices, which is a separate document from this one with its own required contents.

The Notice of Privacy Practices explains how we use and disclose your protected health information for treatment, payment, and operations, and what your rights over it are: to access your record, request corrections, request restrictions, request an accounting of disclosures, and file a complaint.

3. What information we collect

We only collect what you type into our forms, plus basic technical data about the visit. We do not buy contact lists and we do not obtain your data from third parties.

• Appointment request (home page and appointments page): name, phone, email, preferred language, appointment type, preferred date and time, and the reason for the visit you choose to share.
• Insurance verification: name, contact details, and the insurer you are asking about.
• Contact form: name, contact details, and the message you write.
• Job application: name, contact details, role of interest, your message, and the résumé file you attach.
• Technical data we always collect: IP address, browser type, pages visited, and time of visit. We use it to serve the site, for security, and to limit automated form submissions by bots.
• Measurement data: only if you accept the tools described in section 7. If you decline, none is collected.

4. Health information you type into the forms

The reason-for-visit and free-text fields let you write information about your health. We ask you to use judgment: they exist to request an appointment, not to send your medical history.

The information you submit through these forms reaches the clinic's team by email and is treated with the same confidence as any other patient communication. Even so, email is not an end-to-end encrypted channel: please do not include more clinical detail than is needed to book.

This site is not an emergency channel and is not monitored around the clock. If you have a medical emergency, call 911. If you need same-day care, call us at +1 305 364-5778 instead of using the form.

5. What we use your information for, and how we contact you

We use your information to handle the request you made: to schedule or confirm an appointment, verify your insurance coverage, answer your message, or evaluate your job application. We also use aggregate statistics — which do not identify you — to understand what patients need and improve the site.

By submitting a form with your phone number or email, you authorize us to contact you through those channels about that specific request. That consent is limited to exactly that: if we wanted to send you automated text message reminders or promotional communications, we would ask you separately and expressly, and you could decline without it affecting your care.

6. Who we share your information with

We do not sell your information, we do not rent it, and we do not trade it. We do not share it with advertisers and we do not use it to target advertising at you individually.

We share it only with the providers that make the site work, and only as far as needed for the service they provide:

• Vercel, which hosts the site and processes the requests it receives.
• Resend, which delivers the content of the forms you submit to the clinic's team by email.
• Google Analytics and the Meta Pixel, only if you accept measurement and subject to the limits described in section 7.
• Public authorities, when required by law or to protect the rights, safety, or integrity of individuals.

7. Cookies and measurement tools

No measurement tool activates until you accept it. If you have not chosen, or if you choose to decline, this site loads no Google or Meta scripts and leaves no cookies from those platforms on your device. The site works the same either way: booking an appointment, verifying insurance, or writing to us does not depend on you accepting.

If you accept, we use Google Analytics and the Meta Pixel to understand how many people visit the site, which pages are useful to them, and whether our advertising campaigns work. Those tools use cookies or similar identifiers and may record your IP address and the pages you view.

A limit we impose on ourselves: we do not send the content of any form to those tools. They do not receive your name, your phone, your email, your insurer, or your reason for visiting. When you successfully submit a form, we record only that a submission happened and which form it was, with no data about you.

You can change your mind at any time: clear this site's data from your browser settings and we will ask you again. You can also block or delete cookies there at any time.

8. How long we keep your information

We keep what you submit through the forms for as long as needed to handle your request and to maintain the corresponding administrative record. Information that becomes part of your medical record is kept for the periods required by the rules that apply to medical records in Florida.

Résumés are kept for the duration of the hiring process and for any additional period the clinic sets in order to consider you for future openings.

9. Your choices and your rights

Regarding the information you submit through this site, you can ask us what we hold about you, ask us to correct it if it is wrong, or ask us to delete it once it is no longer needed for the purpose you sent it for and no legal obligation requires us to keep it.

Regarding your protected health information, your rights are the ones set out in the HIPAA Notice of Privacy Practices (section 2). Those are broader and include the right to an accounting of disclosures and to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights without any retaliation.

To exercise any of these rights, call us at +1 305 364-5778 or come by the clinic. We will respond within a reasonable time and may ask you to verify your identity before releasing information.

10. How we protect your information

The site is always served over an encrypted connection (HTTPS) and applies technical measures to limit automated submissions and form abuse. Attached files are validated before being accepted.

No system is infallible, which is why we ask that you not submit sensitive information through this site beyond what is needed to book your appointment or ask your question.

11. Minors

This site is intended for adults, and we do not knowingly collect information from children under 13 through it. If you are a parent or guardian and need to book an appointment for a minor, submit the form yourself using your own contact details.

If you believe a child sent us information through this site, call us at +1 305 364-5778 and we will delete it.

12. Links to other sites

This site links to third-party services such as Google Maps, WhatsApp, and the clinic's Google reviews page. Those services have their own privacy policies and we do not control what information they collect. We encourage you to read them.

13. Changes to this policy

If we change this policy, we will publish the updated version on this same page with a new update date. If the change is material, we will highlight it visibly.
